11. Requests for Disclosure, etc.

If you wish to request notification of the purpose of use of your personal information, disclosure of personal information, correction, addition or deletion, or suspension of use or cessation of provision to third parties, or disclosure of third-party provision records, please contact the personal information consultation counter at the end via mail or telephone.

We will send you a 'Request for Disclosure of Personal Information' (hereinafter referred to as 'Disclosure Request'). For requests, please transfer 1,000 yen for each application to the account specified in our request form (transfer fees are to be borne by the requester).

If the fees are insufficient or have not been deposited, we will notify you of this; however, if payment is not made within the prescribed time, we will treat it as if there was no request for disclosure.

 

12. Consequences of Not Providing Personal Information

Providing personal information is optional. If you do not provide some information regarding personal information, we may not be able to respond to your inquiries.
